 Skin issues can progress quickly  if they are not identified early. Individuals with limited mobility,  poor circulation, incontinence, or medical conditions may be at higher  risk for skin breakdown and pressure injuries.Regular observation is an important part of daily care.

Common Areas at Risk

Pressure injuries often develop on:

  • Heels
  • Coccyx
  • Hips
  • Ankles
  • Elbows
  • Back of the head

Redness that does not fade may be an early warning sign.

What DSPs Should Report

Staff should report:

  • Red areas
  • Open skin
  • Blisters
  • Drainage
  • Swelling
  • Warmth
  • Complaints of pain
  • Changes in skin color

Prevention Strategies
